You will then be sent a link to a spreadsheet containing the data collected from your school.

Note: Athletic Directors should inform parents about the use of the online waiver and remind them that the email address they use is considered a digital signature.

Parents/Guardian must use their email address when completing the form. Their waiver is rendered null and void in the absence of this.
